Dachely Hernandez is a family nurse practitioner passionate about helping patients take an active role in their health. She provides comprehensive family practice care for patients age 2 and up. Proud of her Cuban heritage, she is fluent in both English and Spanish, allowing her to connect with and serve a diverse patient population.
Dachely earned her Doctor of Nursing Practice from Clemson University, where her doctoral project focused on developing a three-month CLABSI-centered educational program for medical-surgical nurses. During her doctoral studies, she served as a graduate teaching assistant and clinical instructor at Clemson University – a role that deepened her love for mentoring and education. She also holds a Bachelor of Science in Nursing from Florida State University. Her five-year nursing background has been primarily focused in medical-surgical areas.
Her approach to care is patient-centered and evidence-based, rooted in the belief that empowering patients to take the driver's seat in their own health will lead to better outcomes.
When she's not caring for patients, Dachely enjoys yoga, reading, baking and spending quality time with friends and family.

Hernandez was amazing. She listened to me and asked so many questions. She definitely didn't make me feel rushed. I told her I will definitely recommend her to all my friends.

I understand Dachely is new in her field. She is a very good listener. She and I were able to talk freely and arrive at some conclusions. One important detail that was overlooked was the "After visit summary". There was none. We were discussing how to come off one medication and she said she would investigate and get back to me in the notes. Perhaps there should be a system in place to prompt the provider to do this. I certainly don't think she missed it intentionally! I look forward to working with her in the future.

It was angriest experience with Hernandez! She was caring, concerned and knew all about my medical history and it was my first time seeing her. I will definitely being seeing her when I have need of a medical condition. It was all around a great, comfortable visit.
